Environmental Engineering Failures in Alaska
Insights into past failures in the field of environmental engineering provide considerable value to present-day design engineers and facility owners. Through analysis and study of engineering failures, designers can learn what not to do and thereby improve design, construction, and operations. This results in reduced risk of failure and increased reliability of future projects. Engineering failures can be caused by many factors ranging from a simple case of component failure, which is easily addressed by redundant systems, to more complex situations where individual failures cascade into a catastrophic total system loss involving multiple failures. Engineering failures also can be attributed to multiple factors including judgment errors in the early planning process, engineering design mistakes, defective materials or poor workmanship during construction, or by improper operational procedures. This paper presents several examples of environmental engineering project failures in Alaska and lessons learned from them.
