Collapses usually occur when the parties to the construction process do not have sufficient knowledge or do not understand or ignore their responsibilities. This paper describes the collapse of a trunk storm sewer during construction of a building, due to the improper actions of the design-build contractor and the improper design of the building foundation by the structural engineer.
