Objective: Investigate the peculiarities of osteosynthesis with intramedulary nails and locking plates of femur fractures with damaged lateral wall using 3D virtual surgery planning method. Tasks: Compare osteosynthesis with different design intramedullary nails and locking plate of patients that have 31-A3 (AO/OTA) fracture with type II and III (Cho JW et al., 2017) femoral lateral wall destruction. Methodology: Using 3D virtual surgery planning method proximal part of femur, intramdullary nails with different design A with one support screw that is 11mm diameter and B with two smaller 5.3 mm diameter support screws, locking plate PF-LCP were formed and virtual osteosynthesis was carried out. Randomly 35 patients of LUHS were chosen that had pelvis CT scan done in the past in LUHS. Results: Using PF – LCP with one support screw for 31-A3 (AO/OTA) Type II fracture osteosynthesis resulted in 57% successful fixation. Using 3 screws with the same PF – LCP in type III fracture resulted in 97% successful fixation. There was a greater successfull implantation of both design nails in women group compared to male group (p<0,05) Conclusions: Implantation in 31 – A3 (AO/OTA) fractures with type II lateral wall destruction with B design nail that had two 5.3 mm support screws was more succesful than design A nail with one 11 mm support screw. In type III fractures B design was also more successfully implanted than A design nail. When comparing PF-LCP with intramedulary nails that have one support screw in type III fracture the the PF-LCP had a 100% success rate of implantation compared to 68,6% success rate of intramedullary nail with one support screw.
