The invention discloses a gypsum-based sound insulation leveling adhesion material for a monolithic sound insulation floor slab. The gypsum-based sound insulation leveling adhesion material is prepared from, by mass, 60%-75% of building gypsum, 15%-25% of vibration reduction and sound insulation components, 1%-5% of sound absorption and insulation components, 0.2%-0.6% of stress dispersing components, 4%-10% of strengthening components, 0.4%-1% of viscosity-enhancing auxiliaries, 0.2%-0.5% of water-retention auxiliaries and 0.1%-0.4% of water-resistant auxiliaries. The invention further discloses a preparation method of the gypsum-based sound insulation leveling adhesion material. The gypsum-based sound insulation leveling adhesion material has the good bonding property and deformation resisting capacity and is used for a sound insulation leveling layer of the monolithic sound insulation floor slab, a surface layer can be directly laid on the material according to a common decoration mode, the material can form good bonding with the materials of the floor slab and the surface layer, and the monolithic sound insulation effect is achieved.
