The invention discloses a slurry-leakage-preventing rubber form. The slurry-leakage-preventing rubber form comprises a rectangular frame; grooves with openings in upper ends are respectively formed in four sides of the rectangular frame and are filled with mortar; and the inside of each groove is provided with a reinforcing partition. The slurry-leakage-preventing rubber form is customized according to the position of a grouting sleeve, after the mortar is placed in the rubber form, leakage can be prevented, and a hole of the sleeve can be prevented from being blocked. The slurry-leakage-preventing rubber form is easy to mount and convenient to use.
