The invention relates to the technical field of road intelligent detection and management, and particularly discloses a BIM-based highway pavement disease inspection method, device and system, and the method comprises the following steps: collecting original data; performing end-side analysis and feedback on the original data; remote data transmission; carrying out disease data visualization and early warning; in the fine image acquisition process, after pavement diseases and road mileage pile images are recognized, inspection is stopped, a target disease area and a target mileage pile are locked, and the vehicle is parked at different advancing positions for fine image acquisition; and performing multi-angle shooting on the target disease area and the target mileage pile to obtain a plurality of pieces of image original data with different shooting positions, shooting heights and shooting angles. According to the method, the generation position and the feature information of the pavement disease can be effectively and accurately obtained, data association with the cloud database and the BIM software platform can be effectively carried out, and efficient integration and visual management of the highway detection data are realized.
