A door 102 mounted to frame 101 via hinge 103, e.g. elongate geared hinge, and a hinge cover 104 mounted to the frame. The hinge cover 104 has a first surface abutting the frame and a second surface with a cavity 203 enclosing a portion of the hinge, preferably the hinge cover encloses the top edge of the hinge; the cover may be domed or inclined to provide anti-ligature surface.
