The present invention relates to a device for forming a prefabricated haunch, wherein an inclined surface is formed on part of a concrete structure while forming a mold for forming a concrete structure, comprising: a first form manufactured with a predetermined length to form part of the mold; a second form manufactured with a predetermined length to form part of the mold while being connected to the first form; and a haunch form interposed between the first form and the second form to form an inclined surface of either or both of the first form and the second form while connecting the first form and the second form. The haunch form includes: a main body of a haunch formed with a predetermined length and interposed between the first form and the second form; connection parts formed on both ends in the longitudinal direction of the main body of the haunch, respectively and coupled to the first form and the second form, respectively to be able to be separated; and a bending part formed on a central part of the main body of the haunch to connect the first form and the second form in an inclined state while being bent at a predetermined angle. Therefore, provided is a device for forming a prefabricated haunch, wherein an inclined surface can be formed using an existing standardized form without separately manufacturing a haunch panel for forming a haunch of a concrete structure.
