Die intramedulläre Osteosynthese von dislozierten Klavikulaschaftfrakturen mittels elastisch-stabiler Marknagelung - klinische Analyse, Vergleich der offenen mit geschlossener Reposition sowie mit anderen Therapieverfahren ; Elastic stable intramedullary nailing of dislocated clavicular midshaft fractures by TEN – functional analysis, comparison of open versus closed reduction and with other therapeutic options
Im Rahmen der Dissertation wurden insgesamt 40 Patienten mit singulärer dislozierter Klavikulaschaftfraktur erfolgreich mit elastisch-stabiler Marknagelung behandelt und bezüglich des subjektiven und objektiven klinischen und radiologischen Outcome untersucht. In Übereinstimmung mit den zum Vergleich herangezogenen Publikationen zeigten die Ergebnisse ein gutes bis sehr gutes Outcome hinsichtlich der angewendeten Traumascores (DASH, Constant-Score, VAS) sowie eine signifikant bessere Schmerzreduktion als nach konservativer Therapie oder Plattenosteosynthese. Die Resultate der Scores sich zudem unabhängig von der Art der Reposition, wobei es im Falle der offenen Reposition häufiger zur medialen Dislokation des Marknagels mit Perforation der Haut kam. ; Aim of the study was to compare the outcome of midclavicular fractures using Titanium Elastic Nails with plate fixation and conservative therapy. As assessments were used the Constant score, DASH score, visual pain scale and measuring of muscle force. There have been very good results in function, muscle force and pain-reduction after ESIN, similar to the results after plate fixaion and significant better than after conservative therapy. Most seen complication was the protrusion of the nail after open reduction, which was often necessary in multifragmentary fractures. In these cases, there has to be a critical discussion of the kind of operative treatment.
