The invention discloses a high-color-rendering fluorescent ceramic having a flat-concave structure film and used for laser illumination, and a preparation method thereof. The high-color-rendering fluorescent ceramic comprises a ceramic body, wherein the chemical composition of the ceramic body is (CexY1-x)3Al5O12, x is more than or equal to 0.001 and less than or equal to 0.03, and flat-convex protrusions are arranged on the top surface of the ceramic body; and a film layer body, wherein the top surface of the ceramic body is covered with the film layer body, the bottom surface of the film layer body is provided with flat-concave recesses corresponding to the flat-convex protrusions, the flat-concave recesses are matched with the flat-convex protrusions, and red fluorescent powder is distributed in the film layer body. According to the invention, the problem of non-uniform light spot color caused by non-uniform spatial distribution of blue light in an illumination product can be effectively solved, and the problems of high color temperature, low color rendering and the like caused by insufficient red light components of a YAG:Ce fluorescent material can be improved by the red fluorescent powder doped with the film.
