The invention provides a grout leakage prevention device and method for a concrete formwork. The grout leakage prevention device comprises a soft grout stop ring, an active force application sleeve and a limiting mechanism. The soft grout stop ring is arranged at the steel bar through hole in the outer side of the template; the soft grout stopping ring is arranged on the reserved reinforcing steel bar penetrating through the reinforcing steel bar through hole in a sleeving manner; the active force application sleeve sleeves the periphery of the reserved steel bar; the grout stop ring fixing end is detachably connected with the soft grout stop ring through a detachable fixing mechanism, and a fixing plate is arranged at a port of the limiting connecting end. The limiting mechanism is arranged at the limiting connecting end of the active force application sleeve; the limiting mechanism comprises at least two limiting clamping jaws; the limiting clamping jaws are arranged on the periphery of the reserved steel bar in a surrounding mode. The problems that in the prior art, a grout stopping mode of a water absorption expansion type rubber grout stopping ring is not timely and insufficient and is influenced by a specific construction environment, and the grout stopping effect is not ideal can be solved.
