Based on a visual inspection, operational history, and hydrologic and hydraulic analysis, the dam is considered to be in fair condition. The size classification of the facility is small and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility ranges between the 1/2-PMF (Probable Maximum Flood) and the PMF. Due to the high potential for damage to downstream structures and loss of life that could be associated with a sudden embankment breach, the SDF is considered to be the PMF. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only two percent of the PMF prior to embankment overtopping. The embankment has, in fact, been overtopped at least three times in the past decade. However, due to its unusual configuration, damage has been limited to scouring of the embankment and downstream roadway. Breach analyses performed in this study indicate that there are conditions for floods of less than 1/2-PMF magnitude during which the embankment could possibly fail and result in an increased potential for damage and loss of life downstream. Thus, the spillway system is considered to be seriously inadequate and the facility unsafe, non-emergency. (Author)
