Beyond Bricks and Mortar: Report of Significant Research Findings
The goals were: (1) to build on the NO PLACE LIKE HOME (NPLH) Project begun in both counties with Robert Wood Johnson funds including a special minority outreach effort in Mulnomah County in Subsidized housing. (2) to develop a demonstration project targeted to frail elderly in subsidized housing who are at risk of needing to relocate due to increased needs. (3) to train information and referral volunteers in two rural towns in Clackamas County built on successful FIRST (Family Information Referral Service Team) model already in place in three other rural Clackamas County towns.
