Multiple Related Failures from the Nordic Operating Experience
Abstract The operational experiences from Nordic nuclear power plants are analysed in order to identify dependent and multiple related failures. The components considered in more detail are motor operated valves and diesel generators. The multiple failures are quite few but single failures having causes typical to common cause failures have occurred rather frequently. The analysis of operational experience with respect to other components and common cause initiators have not been performed exhaustively.
