The invention provides a ceiling prefabricated laminated slab with a bottom-mounted double-parallel-type radiation pipe and a forming method of the ceiling prefabricated laminated slab. The ceiling prefabricated laminated slab with the bottom-mounted double-parallel-type radiation pipe comprises a steel bar layer (1), the radiation pipe (2) and a concrete layer (3) which are integrally formed through casting, wherein the radiation pipe (2) is positioned at the lower side of the steel bar layer (1); the concrete layer (3) covers the outside of the steel bar layer (1) and the radiation pipe (2); and the radiation pipe (2) is made into a double-parallel-type structure. The ceiling prefabricated laminated slab with the bottom-mounted double-parallel-type radiation pipe according to the invention has the advantages of simple structure, high heat transfer efficiency and high construction efficiency.
