Fatal Accident Circumstances and Epidemiology (FACE) Report: 22 Year Old Construction Worker Dies When Excavation Collapses
A fatal accident circumstances and epidemiology report is presented. A construction worker died of asphyxiation as a result of an excavation collapse at a housing construction site. Heavy rains had saturated the ground creating problems for the workers at the site. It became necessary to dig a hole in an attempt to drain off the excessive surface water. A workman was in the hole that had been dug to approximately six feet when the side walls collapsed, trapping him under the rain soaked earth. Factors contributing to the accident were the lack of an established, documented safety program, inexperience of the general contractor and worker in shallow excavation work, lack of safeguards to protect the worker, use of hand digging, and working alone. Recommendations include developing a safety program, using shoring in such excavations, and having another person standing by to respond or seek help in case of an emergency.
