The invention relates to the technical field of building construction, and particularly relates to a foundation bolt pre-embedded positioning mold and a foundation bolt mounting method. The foundation bolt pre-embedded positioning mold comprises a center line positioning assembly and a vertical positioning assembly used for being mounted on a foundation steel bar; the center line positioning assembly comprises a positioning form used for being horizontally arranged, a positioning part is formed on the positioning form, the positioning part extends in the first direction, the positioning part is used for being overlapped with a preset center line in the vertical direction, and the first direction is the length direction of the positioning form; the positioning form is used for being mounted on the vertical positioning assembly so as to fix the position of the positioning form in the vertical direction; and first mounting holes are formed in the positioning form, so that the foundation bolts are vertically fixed on the positioning form. The invention aims to provide the foundation bolt pre-embedded positioning mold and the foundation bolt mounting method in order to solve the problem of how to realize precise pre-embedded positioning of the foundation bolts of an ultra-long unit.
