The invention discloses a formwork for a dike slope protection water stop belt. The formwork comprises two connecting formworks which are vertically distributed in a spaced mode and an installation and clamping module connected between the two connecting formworks; the side, in the thickness direction, of each connecting formwork is provided with a pouring space used for pouring concrete; the installation and clamping module is located on the sides, away from the pouring space, of the connecting formworks and comprises a clamping box and an installation assembly used for connecting the clamping box and the connecting formworks; the side, close to the pouring space, of the clamping box is provided with a semicircular rib groove with the cross section matched with a water stop middle rib; an inserting groove allowing a wing piece to stretch into is formed in the groove wall of the side, away from the pouring space, of the rib groove; a clamping assembly used for clamping the wing piece stretching into the inserting groove is arranged in the inserting groove; and the rib groove and the inserting groove are both arranged in the length direction of the water stop belt. The formwork has the effect of improving the installation stability of the water stop belt.
