The invention provides a secondary pre-embedded mounting and positioning device and method for a building water supply and drainage sleeve. The device comprises a support frame, a sleeve positioning assembly, a support template and a sleeve fixing assembly, wherein the casing pipe positioning assembly is arranged on the supporting frame, and the casing pipe positioning assembly extends to the position below the supporting frame; the supporting template is arranged on the sleeve positioning assembly; the sleeve fixing assembly is arranged on the supporting frame. The sleeve positioning assembly is used for positioning the central axis of the sleeve and also used as the positioning reference of the lower-layer sleeve so as to position the centers of the sleeve and the lower-layer sleeve, and the sleeve fixing assembly is used for adjusting the position of the sleeve on the layer based on the position of the sleeve positioning assembly. According to the sleeve positioning assembly, the sleeve on the layer and the sleeve positioning assembly are coaxially arranged, and the sleeve on the layer is clamped and fixed to the adjusted coaxial arrangement position, so that coaxial arrangement positioning between the sleeve on the upper layer and the sleeve on the lower layer is achieved, and the concentricity adjustment accuracy of secondary installation of the sleeve is guaranteed.
