The invention relates to a pulley mechanism of a push-pull door and window, which comprises a shell, a clamping frame, a guide frame, a pulley assembly and an adjusting assembly, the clamping frame and the guide frame are provided with matched clamping guide structures, the pulley assembly, the shell and the guide frame are provided with matched height adjusting guide structures, and the adjusting assembly is connected with the shell and the clamping frame. When the adjusting assembly is adjusted in the forward direction, the clamping frame moves in the direction that the first end of the clamping frame gradually gets close to the first end of the shell, and meanwhile the clamping guide structure guides the clamping frame to generate downward displacement so as to abut against a notch in the lower portion of the door and window sash body. When the adjusting assembly is adjusted in the forward direction, the shell moves in the direction that the first end of the shell gradually gets close to the first end of the clamping frame, and meanwhile the height adjusting guide mechanism guides the pulley assembly to generate downward displacement so as to adjust the height, exposed out of the lower end of the shell, of the pulley. According to the pulley mechanism, clamping installation with a door and window sash body and pulley height adjustment are achieved through forward adjustment of the adjusting assembly.
