PROBLEM TO BE SOLVED: To provide a concrete form that can have a through hole for easy insertion of a material formed, and a method of forming a concrete slab that can secure integration of a closed part where the through hole is closed.SOLUTION: A concrete form 31 for through hole formation which forms a through hole 21 in a floor slab 13 is characterized in that when a direction crossing a first face 13f and a second face 13s of the floor slab 13 where the through hole 21 is opened is a penetration direction, a projection part which projects more inside the through hole 21 than two openings of the through hole 21 in the penetration direction is formed on an inner wall of the through hole 21.SELECTED DRAWING: Figure 1
