The present invention relates to a form to construct a culvert. The present invention comprises: an outer form assembly; and an inner form assembly arranged inside the outer form assembly. The inner form assembly includes: an upper supporting plate; side supporting plates placed on both sides of the upper supporting plate; a hinge unit to hinge the side supporting plates to the upper supporting plate such that the side supporting plates can rotate; a vertically supporting unit arranged on a lower side of the upper supporting plate to vertically support the upper supporting plate, and capable of adjusting a length thereof; and a horizontally moving unit to enable the vertically supporting unit to move horizontally. According to the present invention, an operation is conducted easily and rapidly as culverts in unit sections can continuously be constructed, and separation from the culverts manufactured by curing concrete is easily conducted when the culverts in a construction section are constructed.
