Abstract This paper illustrates the development of a questionnaire to periodically survey a regional distribution infrastructure. The future respondents of the periodic survey were utilized to develop the survey instrument. While many questionnaires are pre-tested and then revised, in this case contact with the panel was initiated to assist with the writing of the questionnaire itself.
