The invention discloses a cast-in-place and prefabrication combined structure for on-bridge rail connection, which is used for realizing rail connection of a newly-built railway and an existing line on the bridge and comprises large frames of a cast-in-place structure and small frames of a prefabricated structure. The cast-in-place large frames are arranged between two adjacent bridge piers of the existing line, the width of the cast-in-place large frames are larger than that of an original existing line bridge, and the cast-in-place large frames are eccentrically arranged towards a newly-built railway on the outer side. The lower portions of the large frames are reinforced through pile foundations, and the upper end faces of the large frames are flush with the upper end faces of the piers. A plurality of prefabricated small frames are transversely arranged on the large frames and arranged between every two adjacent large frames and on the bridge piers in a spanning mode. A newly-built railway, an existing line and a turnout device are arranged above the small frames and used as a bridge of the newly-built railway and the existing line. The method solves the problem of on-bridge rail connection, the construction period can be greatly shortened, the existing line outage time is shortened, and the social cost is reduced.
