The invention discloses a karst goaf steel casing follow-up positioning guide device which comprises a guide support. The hoisting mechanism is arranged on the guide bracket and is used for hoisting the steel casing; the plurality of guide structures are distributed at intervals along the height direction of the guide bracket; the guide structure comprises a first guide rod and a second guide rod which are distributed in a V shape, a guide groove is formed between the first guide rod and the second guide rod in a spaced mode, and the opening side of the guide groove is provided with a steel casing pushing piece and an inner pushing mechanism used for pushing the steel casing pushing piece to move into the guide groove.
