Comprising 360 to 420 parts by weight of cement, 78 to 128 parts by weight of hollow glass beads, 40 parts by weight of silica fume, 20 parts by weight of shale powder, 166 to 225 parts by weight of closed cell perlite sand, 4.85 to 14.55 parts by weight of polyethylene fibers, 10.36 to 13.5 parts by weight of a water reducing agent, 0.54 to 0.6 part by weight of a defoaming agent and 233 to 238 parts by weight of water. A permanent template thus made and a method of making are also provided. The fiber-reinforced ultralight cement-based composite material has the characteristics of low density, high strength and high toughness, and the manufactured permanent formwork has the inherent advantages of the permanent formwork and is thin and light in self weight, so that the construction is simple, the transportation and hoisting cost is reduced, and the fiber-reinforced ultralight cement-based composite material becomes an ideal optimized product of a traditional wood formwork and is suitable for large-scale popularization and application.
