A fixation device 100 comprises an elongate and hollow inner sleeve (102, Fig 2) rotatable about a longitudinal axis 120 and has at least one first cutting element 114 at an open distal end region thereof. A guide body 118 is located on the inner sleeve and has a tapered guide surface which tapers outwardly towards the distal end region. An elongate and hollow outer sleeve 104 is mounted on the inner sleeve and is rotatable about the longitudinal axis and translatable with respect to the inner sleeve. A flareable end formation is located at an open distal end region of the outer sleeve and comprises at least one second cutting element 350. The flareable end formation is configured to flare outwardly towards a deployed position when moved over the tapered guide surface of the guide body. A method of installing such a fixation device in a substrate is also described.
