The invention discloses a grouting treatment method and a system for an underwater karst cave in a karst development area. The method comprises the steps of positioning and hole arrangement, isolation of underground water and a silt layer through a first sleeve, formation of grouting holes through slurry protection wall pipe following drilling, hole washing, replacement of pipe following sleeves through a second sleeve, positioning and installation of a stop-grouting plug, pressurization and fixation, karst cave grouting, grout return pressure monitoring, completion of grouting, hole sealing and grouting effect detection. According to the grouting treatment method and the system for the underwater karst cave in the karst development area, the double sleeves and the stop-grouting plug are adopted for carrying out grouting treatment on the karst cave, and therefore surface water and underwater sludge can be effectively isolated, the grouting hole wall can be stabilized and prevented from collapsing, grouting liquid loss is reduced, and a good protection effect on the environment and water resources is achieved; a novel stop-grouting plug expansion fastening technology is adopted, so that the problems that hole sealing is difficult and the grout return pressure is difficult to control in conventional grouting are solved, the karst cave grouting quality is high, and the filling and reinforcing effects are good; and the grouting pressure can be monitored in real time, grouting parameters are adjusted, integrated construction of drilling, grouting, hole sealing, monitoring and environmental protection is achieved, and creativity is high.
