The middle plate set comprises two end plates fixedly connected to the end of the first baffle piece and two inserting plates detachably connected between an end plate and the end of the second baffle piece. The first baffle piece, the second baffle piece, the two end plates and the two inserting plates surround the end of the prefabricated column, a pouring space is formed among the first baffle piece, the second baffle piece, the two end plates, the inserting plates, the prefabricated column and the end face of an existing column, and the first baffle piece is further connected with a grouting hopper used for guiding concrete pouring.
