The invention discloses a horizontal and vertical combined loading device and method for a pile foundation model test. The horizontal and vertical combined loading device comprises a model box groove formed by welding angle steel and steel plates. The model pile is pre-buried in a soil body in the model box; the side surface of the model box is used for horizontally loading the device A; the upper part of the model box is used for vertically loading a device B; the positions of the horizontal loading pulley and the combined pulley used for vertical loading are adjusted respectively, weights are placed on the corresponding loading components according to test requirements, and the loading test of horizontal, vertical or combined loads of the model pile is achieved. According to the invention, horizontal and vertical loading of the model pile ground can be realized at the same time, when the pile top generates horizontal or vertical displacement, the change of vertical and horizontal loads can be very small and can be ignored, and the bearing character of the pile foundation under the combined load effect can be better analyzed.
