To provide a form plate which dispenses with secondary processing in a construction site, has a sufficient strength while being light-weight and facilitating handling, has good holding performance of a functional medicine such as a termite-proofing agent and a flame retardant, and contributes to foundation heat insulation that suppresses degradation of heat insulation performance, a foundation form and a foundation form connection structure using the form plate, and a foundation construction method.SOLUTION: A surface material 3 is stuck to at least one surface of a plate-like foam resin molding 2, and a plurality of through holes 4 opened in a thickness direction of the foam resin molding 2 together with the surface material 3 are provided at the same height in a longitudinal direction of the foam resin molding 2.SELECTED DRAWING: Figure 4
