To improve workability and work efficiency and shorten a construction period and reduce cost for construction.SOLUTION: A form structure for a columnar structure comprises: a form unit 10 including a combination of a flat-plate buried concrete form plate 30 used for constructing a reinforced-concrete windmill foundation 2 with a polygonal cross-section and disposed on each side surface of the windmill foundation 2 constituting an outer peripheral wall 20 of the windmill foundation 2 and a cross member 40 detachably fixed to the outer surface 30a of the buried concrete form plate 30; and a cross joint member 50 that connects the cross members 40 of the adjacent form units 10.SELECTED DRAWING: Figure 1
