The invention provides a third rail horizontal flatness detection device and a detection method. The third rail horizontal flatness detection device comprises a rail attaching device which is installed on an auxiliary rail lifting device and used for being attached to a detection point steel rail in a pressing mode; the light source emitter is arranged on the C-point detection trolley; the signal receiver and the image recognition device are installed on the rail attaching device, spaced by a preset distance and kept relatively static, and the image recognition device is used for obtaining position image information of the light source emitter on the signal receiver during calibration and working and processing position images. Calculating the relative vertical distance of the light source points in the image under the same coordinate system, and further calculating the relative height difference H between the main line and the turnout branch; and the track lifting control system is connected with the image recognition device and is used for receiving the height difference data and controlling the main track lifting device and the auxiliary track lifting device to execute track lifting on the third track according to the track lifting quantity. According to the third rail transverse flatness detection device and the third rail transverse flatness detection scheme, the transverse flatness detection precision and the detection efficiency are improved, and the detection cost is reduced.
